Melatonin for sleep: dose, timing, and what actually works
Most bottles on drugstore shelves are dosed nowhere near what the research actually tested. Here's the honest, sometimes-contradictory picture from the dose-response literature — including the part where two well-designed research traditions genuinely disagree.
The short version
- Most OTC melatonin (5-10mg) is dosed well above what most trials actually tested. Higher isn't automatically better.
- Two legitimate research traditions disagree on the ideal dose — a low physiological dose (0.3-1mg) versus a moderate clinical dose (2-4mg) — and both have real trial support, for different outcomes.
- Timing may matter as much as dose. The strongest research took melatonin 2-4 hours before bed, not right before lights out.
- It's not a sedative. Melatonin is a timing signal to your body clock, which is why it works gradually rather than knocking you out.
What melatonin actually is
Melatonin isn't a sedative in the way sleep medication is. It's a hormone your brain releases naturally as darkness falls, signaling to your body that it's nighttime — a timing cue, not an off-switch. Supplemental melatonin works by nudging that same signal, which is part of why the dose and timing matter so much more than with something that just knocks you out.
The low-dose case
In a controlled study, doses of just 0.3mg or 1.0mg, taken 2-4 hours before habitual bedtime, significantly decreased how long it took healthy young adults to fall asleep1. The 0.3mg dose is notable for a specific reason: it raised blood melatonin to roughly the same level the body produces on its own at night — a genuinely physiological dose, not a pharmacological one. Follow-up research from the same lab found this same low-dose approach restored more normal sleep patterns specifically in older adults, a group whose natural nighttime melatonin production declines with age2. In a head-to-head comparison, one trial actually found a larger sleep-quality improvement from 0.3mg than from 3mg1 — counterintuitive, but consistent with the idea that more isn't automatically better once you've already restored a natural level.
The higher-dose case
Here's where it gets genuinely contested. The most comprehensive dose-response meta-analysis to date pooled 26 randomized trials (1,689 observations, spanning 1987-2020) and found melatonin's effect on sleep-onset latency and total sleep time kept increasing up to 4mg per day — with no ceiling found in that range3. The same analysis found something most people get backwards: taking melatonin earlier, roughly 3 hours before your desired bedtime, outperformed the common practice of taking it 30 minutes before lights out. Timing, in other words, may matter as much as dose — and most people do both wrong, taking a higher dose than the physiological research supports, right before bed instead of hours earlier.
Reconciling the two
Both bodies of research are real; they're measuring somewhat different things. The low-dose trials focus on restoring a natural nighttime melatonin level, particularly in older adults whose own production has declined — a physiological-replacement model. The dose-response meta-analysis measures the clinical endpoint (minutes of sleep-onset latency shaved off) across a broader mixed population and finds that endpoint keeps improving at higher doses. Neither is wrong; they're different questions. The one point of clear agreement between both: take it earlier than you probably do, and don't assume the 5-10mg gummy on the shelf is better just because it's a bigger number — that dose sits above what either research tradition actually tested as the point of maximum benefit.
What often gets paired with it, and how solid that evidence actually is
Melatonin products rarely ship alone anymore. The most common additions, and an honest read on each:
- L-theanine + GABA — the strongest evidence of this group. A controlled study found the combination reduced sleep latency and meaningfully increased both REM and non-REM sleep compared to either ingredient alone4 — a genuine synergistic effect, not just an additive one.
- Chamomile-derived compounds (apigenin, lemon balm) — plausible but overstated at the isolated-ingredient level. The actual human evidence is for whole chamomile extract, not isolated apigenin: a randomized trial found chamomile extract produced a modest, statistically significant reduction in anxiety symptoms5. Isolated apigenin at typical supplement doses hasn't been tested directly in a human sleep trial — mechanistically reasonable, but a real evidence gap worth knowing about.

Who should be cautious
- Anyone on an SSRI, SNRI or other serotonergic medication, if the product also contains 5-HTP — a real interaction risk, not a theoretical one
- Pregnant or nursing — melatonin crosses into breast milk and hasn't been well studied in pregnancy
- Anyone on blood thinners or immunosuppressants — melatonin has documented interactions with both
- Children — dosing and long-term effects are far less studied than in adults; talk to a pediatrician first
The bottom line
The honest answer isn't a single magic number. A low, physiological dose (0.3-1mg) has real trial evidence, especially for older adults trying to restore a naturally declining melatonin rhythm. A moderate clinical dose (2-4mg) has the strongest dose-response evidence for shaving minutes off sleep onset in the broader population. Both beat the 5-10mg most gummies default to, and both work better taken hours before bed rather than right before it.
